| Volunteer Driver Check List (Complete Volunteer Application First)

August 1st - July 31st, 2019

You must complete the 2018/19 SPS Volunteer Application prior to this form.

Seattle Public Schools believes that field trips provide a valuable educational experience for students. Without the help of volunteer chaperones like you, many field trips would not be possible. We thank you very much for giving your time and support to these important activities. Thank you for your support!
