Driver Check List for Volunteers (Volunteer Application Must Be Submitted First)

You must submit the main SPS Volunteer Application prior to beginning this form.

Seattle Public Schools believes that field trips provide a valuable educational experience for students. Without the help of volunteer drivers like you, many field trips would not be possible. We thank you very much for giving your time and support to these important activities!


Before you begin applying, make sure you have the following items ready:

1. You must carry minimum auto liability limits of $300,000 combined single limit of liability (or $100,000/$300,000 Bodily Injury; $50,000 Property Damage) and uninsured/underinsured motorist coverage. The school cannot approve your application otherwise.

2. Do you anticipate volunteering to drive SPS students more than one day in a school year? If so, please have your three-year comprehensive motor vehicle abstract ready to upload or plan on sharing it with the principal or their field trip designee. Visit WA Dept. of Licensing website to request your full driving records at https://www.dol.wa.gov/driverslicense/drivingrecord.html.

3. Complete the main SPS volunteer application so the information you provide below is linked with your volunteer profile. Please use the same email address at the top of the page to make sure your contact information is connected with your main application.

4. Did your main volunteer application include a clear image of your driver's license? If not, please have it ready for this form or visit the school office and present it to staff.

If your Driving Records show no moving violations in the past 3 years, you use the same vehicle and car insurance and continue to maintain safety requirements outlined below, your volunteer driver approval is valid for 2 years

30. If my vehicle has dual airbags, I will not seat children under 13 or small persons in front passenger seat. *
31. I will not transport students in a motor home, fifth-wheel trailer, cargo compartment of a van or truck bed. *
For volunteers who will drive for more than one day in a school year, including all overnight field trip volunteer drivers.
If you volunteer to drive for an overnight trip, or plan on driving for more than one day trip in a year, you will need to provide your driving records.
